Cost: $20 (Cash or Check made out to Leechburg Area School District)


On Saturday, June 3, 2023, Sports Physicals are being offered for our students by Dr. Daniel Kupas at Leechburg High School starting at 8 AM. The cost is $20 per student.

Attached is the PIAA Physical Form that will need filled out and signed in several sections BEFORE arrival for the student’s physical.

Please sign and return the Leechburg Concussion Information Sheet that has been provided.

You may also have your sports physical performed by your PCP if you are unavailable this day. All forms must be turned in before the sport’s practice start date.

Sports physicals are required for the following sports:

Cheerleading
Basketball
Football
Track
Golf
Softball
Soccer
Baseball
Volleyball
